Care plans are written documents that are in place for each resident. Care plans outline how we deliver safe and effective care for an identified need. For example, a care plan could be written for managing an individual’s diabetes, mobility, hygiene or medication. We start to devise care plans as soon as a resident moves in to one of our homes.
Care plans are person centred documents and are regularly reviewed to reflect a person’s changing needs. We aim to involve the residents and their representatives as much as possible in the care planning process. This ensures that all individuals’ preferences and choices are met. We also aim to deliver care by encouraging independence where possible.
All of our care homes use electronic care plans. This means that the information is kept more readily available, up to date, easy to use and organised. Residents and their representatives can access their personal care plans at any time.
